CHOICES Privacy Statement

CHOICES: Memphis Center for Reproductive Health (MCRH) is committed to protecting patient privacy. Our staff takes pride in providing reproductive health services in a safe and confidential setting making us a preferred choice for reproductive health care in Memphis and the Mid-South.

Our promise to the patient is that we will maintain your confidentiality as long as it does not endanger your life or health. In case of a medical emergency, it will be necessary to release protected health information from your record to a hospital emergency room, another physician, and/or to the emergency contact person listed in your medical record.

Other situations in which it may be necessary to disclose information from your medical record include the following:

If your physician prescribes medication or recommends further testing or treatment by another physician, it may be necessary to disclose protected health information to a pharmacist and/or another physician.

We are required by law to report certain statistical information, for example, the number of abortions performed at CHOICES each year and positive test results for certain sexually transmitted infections.

We reserve the right to collect payment for our services. We have the right to disclose individually identifiable information if it becomes necessary in order to collect our fees.

All persons in the state of Tennessee are required by law to report child abuse and/or neglect. CHOICES does comply with state law and this may include disclosure of individually identifiable information.

We do not file insurance claims or communicate directly with insurance companies. If you file a claim on your own and your insurance company sends a request for information to us, we will mail their request and the information to you at the home address listed in your medical record. It will be your responsibility to forward the information to your insurance company.

We will release protected health information from your medical record if directed to do so by a court order, a subpoena, or if we are otherwise required by law to do so, for example, in a state health department inspection, audit, or investigation.

Records we maintain are the physical property of CHOICES. You have a right to inspect and copy your medical record. You may exercise this right by completing our "Authorization to Release Information" form and remitting a $20 fee. You should notify us in writing if information contained in your medical record needs to be corrected or amended.

You have the right to request an accounting of all disclosures of your health information. Such an accounting will not include internal uses of information for treatment, payment or health care operations, disclosures made to you or at your request, or disclosures made in emergency situations.

If you believe your privacy rights have been violated, you may file a written complaint with the CHOICES Executive Director and/or with the Secretary of Health and Human Services.

You have the right to restrict disclosure of information from your record, but we have the legal right to refuse treatment if you do so. If treatment is refused, you may request a referral to another clinic or physician.

This notice is subject to change. If it does change, you may obtain a copy of the revision at MCRH. MCRH is committed to protecting your privacy.

What is the Escape button?

If you've not already noticed, every page on our website contains an "ESCAPE" button. This button is placed at the top of each page so that if someone comes into the room, you can click this button and immediately be taken to another web page.

We have selected Google.com as this page. When clicked, you will be taken to the main Google page where you can then pretend to be searching for something to avoid scrutiny.

Please be aware that this does not clear your web browser's history, so if the "Back" button is clicked, you will be taken back to whatever page you were viewing before you clicked "ESCAPE." The button is only meant to hide the page you were just viewing from immediate view to protect your privacy.

Clearing Your History/Cache File

If some one knows how to read your computer's history of cache file (automatically saved web pages and graphics), he or she may be able to see information you have viewed on the Internet. You can clear your history or empty your cache file in your browser's settings.
